What is the reason for my sudden, sharp lower back pain?Today while I was driving home from my 4 hour desk job shift, I felt a sudden, sharp pain in my lower right back. It was so painful I had to pullover. Once I did, I felt lightheaded and experienced a shortness of breath. This has NEVER happened to me before, and like I said, I was just driving, not straining or anything. The only other thing I could mention was that I was holding in an urge to urinate (but it wasn't even that bad.) My entire family is out of the country and I have no one else to ask, so any answers would be greatly appreciated! Its been a couple hours now and it just feels sore.

First of all, go to the bathroom if you feel like you have to go. Sometimes you can "reflux" urine back into the kidney and get really bad flank pain. Especially if it was off to one side. Be sure to drink plenty of water and watch for any urinary changes (cloudy, bloody, brown). If they should occur, go see your doctor.

Although you didn't give much information, the situation you are in is something I've seen before. Often people hurt their back while driving their car. Why? Because axial loading (up and down pressure on your spine) causes the discs to push together and pull apart like an accordian. The thing to remember is that the nerves are nearby. If you are sitting just 'a bit off', and hit a slight bump then the discs can't protect your properly. The nerve gets impinged (or 'pinched'), causing pain and probable muscle spasm. The resulting muscle spasm is probably why you're still sore.
Tonight, get on the ice 20 min on/20 min off. Use some ibuprofen if necessary (see label instructions), see the doc if it's still there at all in 2 days. If things get worse or you develop fever/vertigo/nausea go see the doc. Good luck, hope you're well soon.
Story: I had a patient with lower back pain. He'd come into the office and couldn't move. By the time he left, he was feeling great and thrilled with progress. The next day, he was in pain again. Same thing went on for about a week, and we changed everything: diet, exercise, pillow, etc... Eventually we were both getting impatient with his symptoms when I saw him drive into the office in a car with REALLY BAD shocks. Fixed the shocks, no more back pain. Mr. Mechanic was the doctor that day.
